Western Pennsylvania ground is heavy clay over shale, and most lots around here have a slope on them. That combination is why so many walls in this region lean and so many patios settle. Water has to be given somewhere to go, footings have to get below the frost line, and a wall holding back any real height needs to be engineered and drained, not just stacked. That is the part homeowners cannot see and the part that decides whether the work lasts.

Two reasons: water and height. Clay soil holds water, and water behind a wall pushes hard. If a wall was built without proper drainage behind it, or without the right base, it leans and eventually comes apart. A rebuild is more expensive than doing it right the first time.
Yes. Most of the north hills is sloped and a lot of what we do is making a slope usable, whether that is walls and steps, a raised patio, or regrading so water runs away from the house.
